Intravitreal Anti-VEGF: The Drug That Rescued Retinal Vision
A generation ago, wet age-related macular degeneration was a one-way road to legal blindness — you watched the centre of the world dissolve and there was nothing to be done. Then came a class of drugs that does something almost surgical without a scalpel: it reaches into the eye and switches off the single molecule driving the leaky, bleeding new vessels. The result is one of the great therapeutic revolutions of modern medicine — a blinding disease turned into a treatable one. The catch is the delivery: to reach the retina at all, the drug has to be injected through the wall of the eye, again and again, for years.
Steroid Implants and the Pharmacology of Diabetic Retinopathy
Diabetic eye disease is where the retina keeps the score of a systemic illness. Long before a patient notices blurred vision, high sugar has been quietly corroding the smallest vessels at the back of the eye — leaking, then closing, then starving the retina of oxygen. The reflex is to reach for an intravitreal drug, and often that is right. But the deepest truth of this chapter is that the most powerful treatment is not injected into the eye at all: it is good control of glucose, blood pressure and lipids. When drugs are needed, anti-VEGF leads — and for oedema that will not settle, a small sustained-release steroid implant can be planted inside the eye to release medicine for months. That elegance comes at a price the whole of ophthalmology already knows: steroids raise pressure and cloud the lens, and an implant magnifies both.
Dry AMD, Supplements and Gene Therapy: The Retina's Frontier
Anti-VEGF injections rescued the wet form of macular degeneration — the version with leaky, bleeding vessels there is a drug to block. But most macular degeneration is dry, and for a long time dry AMD had no treatment at all: no vessels to seal, no leak to plug, just a slow starving-away of the retina's most precious cells. The frontier of retinal pharmacology is everything anti-VEGF cannot fix. It runs from a humble bottle of vitamins that genuinely slows the disease, to injections that damp an ancient arm of the immune system, to a single gene threaded under the retina that can restore sight a child was born without. The direction of travel is unmistakable: away from injections every few weeks, toward durable, disease-modifying — and even genetic — cures.
